I haven't read through the whole of this, but don't see the need to. Rondo made a correct choice; it's well known that MC tends to ignore the caps rule. If MC can't abide by that, respect another OP's word, and do as asked of him, regardless of his history as an OP it does call for punishment. Just to confirm, I can understand why you'd think that several OPs would need to agree in this instance, knowing MC has had history as an OP, but as said by Clown and others, it isn't necessary if the reasons for banning are clearly justified. Really, if there is good reason for a ban, and proof is provided, then any arguments/ debates about whether the ban was deserved or not can be made here in the appeals section.
Nevertheless, MC has been warned several times, and although it isn't severe, he can't just keep acting like this when asked something as simple as to stop talking in all capitals. And TFG, if MC wishes to be unbanned, he'll have to make an exception to not using forums. Unless there is a genuine reason for him not using forums, such as not being allowed to by parents, then it's rare that we will take someone elses word as his own. We need to know that he'll have the commitment, and take the effort to ask to be unbanned. As Cole has said, we believe in forgiveness, but not without an apology from that player. If they "do not care" about the reasons for why they've been banned, why should we unban at all?
Really, we just want MC to know that we are not overly happy with him continually breaking this caps rule, and being disrespectful. If he shows he is sorry, apologises here himself, and tries to make an effort to not behave this way so much in the future, there's no reason that he can't be unbanned.
Other players may not completely agree with him being unbanned so easily, but I don't think anyone can deny that if he does show reform, then there's no reason for keeping him banned. Whether his attitude and behaviour does improve however is up to him.
Edit- I didn't see the other two pages after the first one, still think my points are valid though, lol. A month ban seems unfair really to get the message accross, I personally still think a forum account should be made though, if there is no strong reason MC doesn't use forums.
